Referral Partner Highlight: Health Care for Re-entry Veterans (HCRV) program
The Health Care for Re-entry Veterans (HCRV) program is designed to help incarcerated veterans successfully reintegrate back into the community post-release by providing resources and support while incarcerated, to successfully plan for re-entry. The HCRV program’s mission is to prevent veterans from becoming homeless once reintegrated back into the community and optimize chances for successful community reintegration and readjustment through comprehensive psychosocial services and transition planning. HCRV achieves this by providing outreach and pre-release assessment services, referrals and linkages to medical, mental health and employment services. This program provides the important link that veterans who are preparing for re-entry often need to develop a successful plan toward stable futures.
